Congratulations on earning your monthly challenge shirt. In order to make sure you receive your shirt, please submit the form below. We will notify you via email when your shirt request has been sent to our printing vendor and then again when your shirt is ready to pick up at Madison Social. In order to receive your shirt, you must present a completed monthly challenge card upon pickup. Thanks.
